Give me insight that I may better appreciate the love that I am so fortunate to share with so many, and help me find a sense of direction in my life that uses the talents you have so graciously bestowed upon me in the most positive, loving and supportive ways."
Given this beginning prayer, you can simply expand it. Talk about your strengths and weaknesses. Be specific about your good and useful traits as well as those things that hamper you from doing your best. Tell how you plan to optimize your strengths and what you might do to confront your weaknesses. Be as honest and straight forward as you can be.
Now, what would God write back? First, He would thank you for coming to him in all your humble sincerity. He would say how much he understands and appreciates your request. Then he would tell you that the answers to all of your questions can be found within you because that is where God resides. It is you who are responsible for the work you do to improve yourself. And the best time to get started is the ever-present now.
The prayer at the end need only restate what you have already asked for in the beginning but with the added understanding that you alone are responsible for the things you do and the positive choices you make.
